Attendee types: Registrars | Medical Officers | Ophthalmic personnel: Allied Health, Nurses, Ocularists, Orthoptists, Administrators, Assistants, Managers or Optometrists.
10% Group registration discount apply to groups of 6 or more Ophthalmic Personnel (not for Ophthalmologists) from the same practice.

Should you require a proforma invoice in order to arrange payment, then please send a note at the time when you submit your form.
